What a Form 483 is, and why it matters
Form FDA 483, “Inspectional Observations”, is issued at the close of an FDA inspection when the investigator saw conditions that, in their judgment, may violate the Federal Food, Drug, and Cosmetic Act or related regulations. Each numbered observation describes what was seen, often quoting the requirement in 21 CFR, such as 211.192 for production record review or 820.100 for CAPA.
A 483 is not FDA’s final decision. The firm usually answers within 15 business days, and FDA weighs the response, the inspection report and the evidence before deciding whether to close the matter, ask for more, or escalate to a warning letter.
For quality, regulatory and supplier teams, 483s are the most direct evidence of what investigators look for: which systems they test, how they word a citation, and which problems keep coming back at sites like yours.

Questions
What is an FDA Form 483?
A Form 483 lists the inspectional observations an FDA investigator gives a firm’s management at the end of an inspection, when they saw conditions that may violate the Food, Drug, and Cosmetic Act. It is not FDA’s final determination of a violation.
How long does a firm have to respond to a 483?
FDA asks for a written response within 15 business days. A response sent in that window is considered before FDA decides on further action, such as a warning letter.
Does a 483 lead to a warning letter?
Not always. Many inspections end with the 483 and a satisfactory response. A warning letter usually follows when FDA judges the violations significant and the response inadequate.
What is the difference between a 483 and an EIR?
The 483 is the short list of observations handed over at the end of the inspection. The Establishment Inspection Report (EIR) is the investigator’s full narrative, written afterwards.
